Stop Smoking

We looked all over the department when we found the room empty. With a history of throat cancer, and a complaint of coughing up blood, we decided she needed to be seen sooner rather than later. We checked with a nurse who said she went into the fast track portion of the ER. This is were simple complaints like sore throats and coughs went– a basic “urgent care center” area of our department. Another nurse said she had been moved to a different room. We found nobody. After a few moments, we found her in her assigned room. It had taken triage half a life time to bring her back.

She looked every bit of sixty five plus twenty additional years. She was frail and held a wad of paper towels up to her mouth. The toweling had flowers imprinted on them mixed with spots and blotches of bright red blood. Her son, a strange looking long haired fellow, accompanied her. He wore a look of concern mixed with an expression of experience. He had been caring for his mother and had watched her deteriorate in front of his eyes for the last year. He had taken her to all of her doctor’s appointments. He was there when the surgeon told them how uncertain he was regarding the removal of her extensive throat cancer. He was there before then when she was diagnosed with breast cancer, colon cancer, lung cancer, and when she was diagnosed with this new aggressive beast consuming the anatomy of her throat.

It had devoured her larynx, leaving in ruins the voice which had scolded him, praised him or advised him as he grew from infant to man. She communicated through a whisper which was easily overwhelmed by the sound of a gurney wheeling by, a page overhead, a door being closed, the air conditioning kicking on and off. The tumor added girth to what should have been a slender neck. She had the neck of a woman three times her 88 lbs.

She coughed to clear her airway, holding the tissue to her mouth, as she sat on the side of the bed. “I’ve talked to Dr. B about the surgery and…” She was cut off as her body’s instinct to protect its airway took over.
“She decided against the surgery,” the son, who leaned casually against the wall next to the otoscope, said for her.
In a whisper, she said, “I’m not sure if I want to try the surgery or if I just want to go gracefully. This certainly wasn’t worth the cigarettes,” she chuckled weakly. Between she and her son, it was explained that the bleeding had started in the morning and hadn’t stopped. Her color was good, for a patient dieing of cancer. She was in good spirits despite blood running down her throat.
“I don’t see any active bleeding,” Dr. M said after examining her. “I want to order a blood test and speak with Dr. B before we let you go.” She nodded, her son nodded, and as the doctor and I left, he said to me, “Get an H & H on her and have Dr. B paged” after dictating the rest of the exam out of earshot of the patient.

We spoke with her doctor who said this was the natural progression of the tumor. He stated no major vessels were involved, so she could go home. We talked him into seeing the patient in the department, which he did. Fifteen minutes after he left, a nurse got us. “Her bleeding is worse. She’s just spitting blood.” It wasn’t an exaggeration. We found her over the sink with her son holding her up. We hoped she was vomiting blood she may have swallowed, but something wasn’t quite right.
“Lets suction her,” the doctor ordered and we began clearing her airway. We laid her down in bed, continuing to suction as needed between talking to her and her son. It happened so quick, it’s a blur. She stiffened, lost all color, and became unresponsive. A quick assessment showed us she wasn’t breathing. “We’ve got a code,” the doctor announced and we bagged her quickly. She still had a pulse which was good.

We quickly moved her to a trauma room. A team had assembled, meeting us as we rushed her down the hall with her son jogging behind us. A paramedic walking his stretcher to the ambulance stopped to give us the right of way.
“Shit,” the doctor muttered twice as we pushed her into the big room. We quickly accessed her veins to pump fluids into her while her son stood outside the door. The doctor had his tools in hand to intubate her, to provide life supporting oxygen.
“Suction her and bag’er,” he said. Respiratory had just arrived and took over bagging after the doctor suctioned copious amounts of blood from her airway. The doctor placed the tube, taking a moment longer than it usually took him. “Have we got a pulse,” he asked as he listened to her chest. He quickly shook his head, “We’re in her stomach. Lets try again.” Because of the tumor, her airway was obstructed. Blood was pouring everywhere which further complicated the doctor’s ability to visualize her vocal chords.
A nurse shouted, “She’s brady down in the 30′s!”
“Start CPR,” the doctor ordered and searched through some tools. “Lets give her one of Epi and one of Atropine!” He selected another tube and attempted again.
Again, air was being pumped into her stomach instead of her lungs. “Lets check a pulse,” the doctor asked. “Is she on the monitor? We need her on the monitor!” He glanced over at a tech and barked, “We need pads on her! Get the pads on her now!” To another nurse, he said, “Lets release that feeding tube to keep her from puking.” Air whooshed from the tube as the nurse did so, and her stomach noticeably came down.
With a new long flexible hose, the doctor again attempted to intubate her, halting CPR to do so. Again, the hose snaked around because of the mass and distorted anatomy from that ugly beast, the tube was in her esophagus. “Goddamnit,” he growled and continued to attempt to get her some air.
“I have asystole on the monitor,” a nurse announced. Asystole is flat line.
“Continue CPR,” the doctor said, ripping off his gloves. “Let’s give her another epi.” He left the room to get permission from the son to cut open her neck and intubate her that way. Or, to discuss what the son’s wishes were.

The son stood with disbelief shining through wide eyes. His hands were in his pockets and a social worker had already come to his side. He looked up as the doctor greeted him under such grim conditions. “I can’t intubate her because of her mass. What I can do, what I would need your permission for, is to go through her neck. I know you mentioned she had expressed her wishes should something like this come up. How far do you want to go?”
I could tell the son had his hands in his pockets to hide their trembling. Even so, I could see the tremble in his hands and in his shoulders. “She didn’t want to be on life support,” he said, his voice shaking. “She didn’t want to have the neck thing done.”
The doctor nodded. “Okay. I’ll see what I can do,” he said to him. “I may not be able to save her if I can’t intubate her, you know.”
The son nodded with understanding, “It’s what she wanted. She didn’t want a big effort.”
The doctor nodded and gave two more tries of intubating without luck. Defeated by a clump of tissue, he looked at everyone, “Any suggestions that anyone would like to make?” Nobody spoke. He waved his hands to the silence. “Lets call it. Clean her up.”

The son didn’t break down or fall apart once given the official news. He stood trembling with wide eyes. He didn’t realize these were his last few moments with his mother. I think later he’ll realize it was a good thing he was there with her, that she didn’t die alone, and that he was selfless enough to respect her wishes. It was a somewhat ugly way to go, but it could have been worse. Much worse.
I saw him walking down the hall, probably leaving to make funeral arrangements. It would be his first steps out into the world without his mother in his life. It must be odd to walk in with someone, and leave without them. I watched him as he rounded a corner, disappearing from my sight. I silently wished him the best. I wondered if he was now free to live his life without having to care so much for his mother. Was he devastated by her death? Would he be relieved? How harsh would the sun be as he went outside, or would it be soothing to him? And I wonder how different things might have been for him had she never had a single puff off of a cigarette.

Something Happened in California

It was a painfully slow day. There was a football game on TV which always clears out the ER. At that time there were 2 docs on shift. The doc and I weren’t the latest shift there. In fact, we were fixing to move upstairs to the fast track section of the ER. It’s where all the runny noses, watery eyes, and finger cuts go. They don’t require the big workups the patients down in the ER will need. We set the fast track people in a room, throw some meds at them and if some stick in their mouth, we discharge them with a 1-2 day work/school excuse.

The room we were gathered in, due to construction, had plans to be divided into multiple rooms. I had gone over the dimensions and had mentally calculated the best way for them to divide up the rooms to give them the most usable space.
We were *that* bored.
The doc let out a painful groan, stared into his coffee cup, then looked at me. I shook my head. No, no new labs were back. He didn’t have to ask, I didn’t have to say.

Moments later, two new patients popped up as needing to be seen. One was an old lady with a head injury. Another was a 59 y/o lady complaining of depression.
I showed the doctor. He looked and asked, “Which should we see?” He was to the point he didn’t care. Such slow days suck the life out of you…much worse than the busiest hardest days. I looked at the clock and realized we were to go upstairs soon.

I rested my chin in my hand and looked over their past medical problems and meds. Yikes! Head injury lady was on Coumadin and would need a big workup, not to mention a CT scan. I chose depression lady and told the doc.
He nodded and took a few sips of coffee before grabbing his stethoscope and standing up, reaching towards the ceiling in a stretch. “If she takes forever, it’s your fault,” he warned me.

The woman sat in a chair in the room, not on the bed. Her legs were crossed, her arms were folded. She certainly didn’t look happy.
The doc starts with the typical, “What brings you to the ER?”
She looks up and I think, “oh dear”. She reminded me of my step mother. Bat-shit-total-loon with a side order of bible thumping crazy (no offense to the religious folk).
I parked it across the room from her. The nurse stood next to me. I gathered she had the same sense. The doc, well, he doesn’t care about that stuff and sat on the exam table across from her.
“My shoulder hurts,” she tells us. Immediately I recall a suicide attempt, reason being because the guy had a bladder infection and thought this would be similar.
“Your shoulder hurting is why you’re here,” the doc questioned.
“Well, it all started with my shoulder,” she said to us indicating that it was story time. Oh joy! Story time!
“A screw came loose,” when she said it, I tried not to laugh at that, “and the surgeon refused…REFUSED to fix it.”
“Okay,” the doctor said.
“I kept telling him that it hurt,” she said, tears filling her eyes, “but he wouldn’t do a damn thing.” She stabbed her finger towards the floor with each word, “Not a damn thing.”
“Uh-huh,” said the doc.
“And I’m worried about my son. Since something happened in California.” She looked at the doc. We all knew what she wanted, including the doc. She wanted him to ask what happened, but he wasn’t about to open that can of worms.
“My son hasn’t been right ever since.” She wiped her eyes and twitched her foot. She was wearing Keds. They were plain white. She had some green pants on with a purple shirt and a grey hoodie over it. Her pants rustled when she kicked her foot. She still had her arms crossed and wore a scowl on her face. Her lip quivered, “So I just don’t know what to do. I used to see a psychiatrist before something happened in California.”
“When was the last time you saw them,” the doc asked her.
“A year ago. That was before I had to get out of California.” She looked at him again. I could see it in her eyes. She wanted him to ask so badly it was almost painful to her.
“Were you on medication,” he asked her.
She nodded and kicked her foot again since he didn’t ask about California. “I was on several things for depression. None of them worked.”
The doctor asked a few more questions then said to her, “Let me tell you how things will go. I’m the emergency physician. I don’t have training in psychiatry so all I will be doing is just clearing you medically. Then we’ll have a social worker come to talk to you–they have more training in psychology than I do–and they’ll set you up with a psychiatrist.”
“That’s fine,” she said to him. She covered her face after a sob, “I just need to talk to somebody. What I’ve been through is horrible.”
“We’ll get someone for you to talk to,” he said to her and moved to begin conducting an exam.
She halted him by raising her hand. “When I was in California,” she started. I thought, oh boy, here it comes. I was interested actually. She made me curious. “When I was in California, Homeland Security came into my house…” she paused as her voice began to shake. I was mentally shouting for joy. The crazy gods had smiled down upon us for she is dragging the government into this! “…they ripped me from my house and took me away…” Again, she had to pause. Yes, yes! Go on! “They threw me into some hospital and drugged me with sodium penthol.” Government and hospitals and drugs, oh my! This is turning out to be a great story time! Everyone in the room was absolutely still. We figured she was about to snap and we didn’t want any vibrations from our movement to cause her to finally let go.
She let such atrocities sink into us before continuing. “And after that, they returned me to my house like nothing happened.” She wore a mock expression of amazement. We were all also amazed. This was the perfect crazy person story. And all she needed to do was snap.

“Okay,” the doctor said. …and snap! I could almost hear it.

The snap manifested on her red face. “No! Don’t tell me it’s ‘okay’! Don’t dismiss me passively!” She pointed to him and shouted, “What if it was your mother, your daughter, your brother, your grandmother, your aunt, your uncle, your wife!” For a while, I didn’t think she’d stop and I wondered if she would include me into her rant, but she didn’t.  My self esteem took another blow.  “Don’t take that passive…” she was so flustered she couldn’t finish and flapped her hand at him. “Just, get out! I don’t want to talk to you anymore! You can just send in your little social worker!”

She wanted a rise out of him. Or maybe she wanted him to drop to his knees and beg her to let him stay. But not this doc. He would gladly leave. He would even grab himself by the back of his scrubs and fling himself out.
“Okay, we’ll send in the social worker,” he said. I could almost hear the joy in his tone.
We left. I looked to him and said, “I didn’t see a tinfoil hat on her head. Do you think it’s in her pocket or is it invisible so Homeland Security can’t detect it?”
He chuckled. He made a space of about an inch between his thumb and index finger and said, “She’s just a little delusional.”
“But you got out of there fast,” I said to him.
“I appreciate that.”

Later I looked at her shoulder x-ray. What do ya know…she had a screw loose.

Scared Clean

You could hear the coffee maker gurgle on this side of the ER amongst the beeps of monitors happily watching their patients and employees talking between themselves about their weekends. The smell of fresh coffee wafted through the air which is always a welcomed smell. At times you only smelled urine and the peppermint scented cleaner. Make no mistake though, that pleasant smelling coffee could burn a hole in your stomach the moment it slides down your throat. Even with this information, our nares were enjoying the break from offensive odors.
It was nice and calm, which is rare AND odd. We get a little suspicious when it’s quiet and calm. That is why we were able to relax when Mr. V became upset and started yelling, disrupting the unnatural silence of the ER.
“What the hell do you mean,” he growled with a reddening face. “I’m gonna sue your ass for not treating me! You fuckers don’t know your ass with both hands!” He was so upset, he messed up the insult he venomously spat at Dr. M.
But Dr. M stood ramrod straight with a passive look on his face. Mr. V, a slim forty something year old male with silver temples that stood out against his black hair, was a frequent flier and had been marked as a possible narcotics abuser. Dr. M was on the end of this verbal assault for telling him he would not be receiving any narcotic drugs. Mr. V already had received a “pain letter” from our department which tells the patient “no more narcotics”. I looked at his chart. Mr. V claims he’s allergic to all pain meds except for Dilaudid.
Yeah right.

He was also in for abdominal pain which can be a tricky area of the body. Many drug abusers know to use this complaint because of it. We usually CT scan them which, as they may not know, the contrast is hell on the kidneys not to mention the awful amount of radiation their belly will absorb–the equivalent of 500 x-rays in one single scan.
He was in two weeks ago for the same thing…and two weeks before that…and two weeks before that. Every two weeks or so, for the past few months, Mr. V has been in for abdominal pain and we’ve been unable to determine the cause. His loud outburst to our refusal to provide narcotics is only further evidence he’s abusing.
And to think there are people in the waiting room with real emergencies, waiting, because Mr. V wants his Dilaudid.

We knew of his potential outbursts, yet somehow he was in a room with a view. Only curtains surrounded him which let the entire department hear his outbursts. Dr. M took his verbal beating without even flinching. This was typical drug abuser verbal diarrhea. He’s heard it all before. Once Mr. V took a breath, Dr. M stated, “Say what you want, but I’m not giving you any narcotics. You can have Tylenol…”
“Fuck your Tylenol. I want my lawyer,” Mr. V carried on, waving his arms like mad. Dr. M turned and rolled his eyes while Mr. V continued, not caring that Dr. M was walking away from him.

We met Mr. C in the hall as he was being transfered to the cath lab for treatment of his recent heart attack. He looked at little pale, but smiled and waved as the tech pushed his bed. Then his hand dropped. It was an unnatural drop and the monitor screamed for attention at the same time. Mr. C was in v-fib.

In two large strides, Dr. M was at his side. “Lets go,” he yelled to no one in particular. He raised a giant fist while paddles were readied and slammed it into Mr. C’s chest with a sickening impact. The monitor didn’t change after the precordial thump. As readied paddles were handed to Dr. M, he announced the voltage, “Clear,” and shocked Mr. C back to life right there in the hallway for everyone to see. Everyone smiled since most codes don’t end with a live victim. Even though we were out in the open, there was only one witness.
I looked down the hall and saw Mr. V craning his neck to see. He sat wide-eyed and silent, staring at the bed where Mr. C now started to comfort the area where Dr. M had hit him.

Mr. V silently eloped from our department. We haven’t seen him for months. Maybe he’s going to another department for drugs? Maybe he’s dead. Perhaps what he saw jolted him badly enough to realize what he was doing. Hopefully so, but probably not.

Respiratory Duress

We had just returned to the ER from the ICU after an attempt to bring a dead person back to life. It was disheartening to learn that the patient was scheduled to move out of the ICU and to the regular medical floor before developing an internal bleed. CT results had been resulted only minutes before his heart stopped beating.
It was our third code of the day and everyone was beginning to feel the effects. Things were beginning to back up. Patients had been waiting longer than what was typical and there wasn’t a whole lot we could do about it.
After going through labs and ordering new labs on those who needed it, discharging those we could, we went to see something ridiculous. I looked at the chart then at the doc. “Hair loss. Is this a joke?”
The doc rubbed his forehead and opened his hand in the air. He scoffed and said, “Probably not.”
The young man slowly removed his 49′ers baseball cap and pointed to a 3 inch diameter circle of baldness on top of his scalp. “It fell out this morning.”
The doctor merely glanced at it from a distance. “It’s a yeast infection.”
The patient’s eyes widened in disbelief then he sighed with relief. “Oh. So it’s not an aneurysm?”
Annoyed, the doctor flatly said, “No. Use Selsun Blue shampoo. If it doesn’t clear up, see your primary care doctor because this isn’t an emergency. Any more questions?”
Sensing the doctor’s mood, the patient shook his head and we left the room. The doctor only had a moment to rake a hand through his hair before a minivan wildly pulled up into the ambulance entry. A tech and a nurse met them outside and while returning with a stretcher, the tech called out, “I think this is going to be a code.”
“Yay,” the doctor muttered unenthusiastically.
The nurse, a big guy, carried the patient into the trauma room where we waited. There was no time to dilly-dally with a stretcher. By the time she had been moved from the van to the trauma room, she stopped breathing. A tech positioned his hands for compressions while a mask and bag was placed on her face. The doctor glared at him and asked, “Does she have a pulse? Have you checked for a pulse?”
“She has a pulse,” the nurse seemed shocked and the tech busied himself with other tasks, other than compressions.
“ABC’s, people. ABC’s,” the doc’s sharp voice had the effect of a whip being cracked. The mantra, “If you don’t have an airway, nothing else matters” rang in my head from an old instructor I had.

I noted the patient’s swollen legs. Her calves were as large as her thighs due to the amount of swelling. Her skin was so tight it was shiny. I expected a medical history of heart failure. By the time her husband returned to the department, we had her intubated, continuous monitoring, radiology was on their way with a STAT chest X-ray, blood work was started, and respiratory was getting ready to take over her breathing. Her husband peeked through the door of the trauma room and I waved him in.
From the head of the bed, the doc looked up. “She’s got a pulse, but she’s not breathing on her own so we’re breathing for her. Her blood pressure is good, her oxygen saturations have improved. Tell me what happened.”
The husband was a tall strong looking man with silver hair. His eyes remained on his wife and glanced at the doctor only a couple of times. He shifted his weight back and forth on his feet as he spoke. “She wasn’t feeling good this morning and said she was having trouble breathing. She has pneumonia…”
“How long has she had pneumonia,” the doctor interrupted.
The husband wrung his hands, “Three weeks? She was at the doctor just a few days ago and said she sounded better.”
“What medications is she on?”
“Um…some type of…I can’t remember,” he scratched his head as he thought. “I didn’t think to bring it with me. She started falling forward in the car and…”
“Does she have any other medical history,” the doctor asked, not allowing the husband to crumble at this very moment.
“High blood pressure, I think. She just wasn’t feeling good…”
“Who’s her doctor,” the doc asked. The husband gave the name to someone the ER doc had never heard of.
The patient was on the ventilator by the time the husband was finished being questioned. She was stable enough that most of the team was able to clear out and allow the husband some time with his wife. Radiology had already shot a picture of her lungs and looking at it, comparing it with her blood work, this was congestive heart failure.

We could hear the husband speaking to his wife. We had given him a chair to sit in and he rubbed her arm. “Why didn’t you listen to me? You wouldn’t be like this if you listened,” he said, and rubbed his eyes on his sleeve. His adam’s apple bobbed and I watched a mountain of a man slump and sob once before begging his wife, “Please don’t leave me yet. We’re supposed to go to France this summer, remember? God, don’t let her leave.” His face was red and had tightened with grief.
A lump formed in my throat and I had to turn away. I silently wished his stubborn wife to listen to reason. She had seen some weirdo alternative medicine person, who misrepresented their self as a medical doctor for a PhD, who didn’t have the medical training to adequately treat her, who stated that their diplomas were hung in the waiting room so there was no misrepresentation at all. The name of the office made it sound like a clinic and not some hippy quack experimental lab that it was. Alternative, or experimental methods have their place. But damn anyone who represents their self as anything but. Whether or not the patient deserved to get into a little bit of a pickle regarding her lack of researching someone providing her healthcare is arguable. But her husband certainly didn’t deserve this stress. He deserved to be out in the sun caring for a lawn he might love, playing with his grandchildren, vacationing in France.

We were able to remove her from the ventilator fairly quickly. She was awake and conversational by the time she went upstairs to her hospital room. The husband looked to have aged 5 years. Not only did he shake the doctor’s hand, but he thanked the entire team for saving his wife. He was certainly a very kind and likable man, one we hope will get to enjoy the rest of his life rather than being tied to a hospital bed, watching his wife.

The Future of America

When we get a 14 year old female in the ER for abdominal cramping and bleeding, we pretty much already know what it is before walking into the room. But this one…

The doc and I walk in and, as usual, the first thing out of his mouth is, “Hi, so what’s going on today,” as an opening. It’s to get the patient or family/friend/whoever they brought with them to open their mouth and start in about why they’re here in the ER. Usually it works well. There are a few occasions where people seem to be confused by greetings.
This girl and the older woman with her both gave us blank stares.
“What brings you to the ER today,” he tries again. The two look at each other then look back at us. This particular doctor I was working with has a temper as short as your pinky finger and I can already see that he’s starting to boil.
Slowly, and with more annunciation than needed, he said, “What… brought… you… to… the… emergency… department?”
It finally dawned on the older woman what it was he was asking and she confidently answered, “Oh, the ambalance brought her.”

Oh boy.

“No, why…” The doctor paused then decided to roll with her answer. “Why did you call the ambulance?”
The older woman gestures to the patient and we discover the patient does, in fact, know how to speak. She says, “I went to da baffroom and had blood.”
The doctor asked, careful to use simple terms since she acted like she was 9 years old, “When you went pee or poo?” He didn’t want to confuse them with terms such as “urinate” or “bowel movement”.
“I had diarrear and had blood,” the 14 year old said to him.
“So you had blood with your stool?”
“I went pee alot and had blood,” she said.
So is she too stupid to know if she took a shit or if she took a piss?

The doctor is frustrated and asks, “So did you go pee or poo? Which is it?” The 14 year old gives him a blank stare. He turns to the older woman, his frustration tangible, and asks exactly what I’m thinking, “Is she retarded?” The 14 year old’s eyes looked like they were about to fall out of her head upon hearing that and the older woman shook her head in a not-quite-so-sure manner. “Are you her mother,” the doc asked.
“No, I’m her momma’s best friend.” Odd, I think, that Mom wouldn’t come with her to the ER.
The doctor gives up and pushes on the patient’s stomach who squirms and tells him, “Ouch!” multiple times. Her brow is furrowed down. She’s mad at him now.

“Roll over,” he orders and the nurse helps her though the patient isn’t quite so sure about this. She had no choice and the doctor looks at her rectum. No blood. She gets rolled back over and he looks at her vagina. There’s blood and she’s quite offended by the exam.
“When was your last period,” the doctor asks.
Pissed off, she tells him, “I dunno. It been three, fo’ months.”
“Did you think to take a pregnancy test,” the doctor asked.
“What,” she looks at him with equal parts fury and confusion.
He takes note of her anger and says, “Well, if you don’t want people looking at you, you shouldn’t be having sex. You’re probably pregnant,” he tells her. When her expression changed to shock he added, “You haven’t had a period in 4 months and you have cramping… I mean what did you think it was?”

She returned to being angry. She had enough neurons firing to pick up the fact he was treating her like an idiot and nearly making a show of it.
She also smoked, drank, and did cocaine… at 14 years of age. Yep, there’s a lovely breeding member of America we’ll be supporting.

Her ultrasound showed she had fetal heart tones and a live IUP, but severely decreased amniotic fluid. She won herself admission to the hospital and a D & C… I’m sure on all of our dimes.

As we left the room after the exam, the doctor said rather loud enough for them to hear, “I don’t think they have two complete brain cells between the both of them.”

Futile Fights

His buddy was in because his nipple hurt. He recently had a nipple ring put in but, as he explained with a tone laced with past experience, “This crazy girl ripped it out”. It abscessed, became painful, we drained it and discharged him to parking lot.
The buddy comes to pick up his friend and is dressed part vanilla ice and part flaming hoochie man. So the girl who ripped out the nipple ring might actually be this guy, but I don’t know. His pants were ten sizes too large, he wore a tight net tank top, and his hair was bleached and damaged from, what I assume to be, multiple Dennis Rodman worshipings.

I’m sitting in a little nook typing up instructions for another patient and hear the nurse yell, “Pull your pants up!” She has to repeat herself multiple times because I guess those instructions were not clear enough the first time.
“If you don’t pull your pants up, I’ll call security. This is a public place! You do not need to be exposing yourself! There are children in here for God’s sakes,” she continued.
She has to repeat this again. I look up while gathering what I printed and see who it is. It was just in time to see the nurse tell him, “I don’t wanna see it! Pull your pants up!”
He holds his hands to frame is groin area, “Why you be lookin’ if you don’t wanna see it,” and rolls his neck with ‘attitude.
“Pull…your pants…up,” she demands. “I’m calling security.”
“Why you be lookin’ den,” he hollers at her, throwing his hands out to the side in a provoking gesture.
I looked, out of curiosity to see how far down his pants really were, and from my point of view I got a glorious booty shot. This had to be on purpose because that was a big booty.  As baggy as those pants were, they didn’t just slide down that bubble. He yanked those suckers down.
“Your underwear is showing and your butt is hanging out,” the nurse scolded while holding the phone to her ear. “This is a public place…yes, I’ve got a man up here who refuses to pull his pants up and is getting combative about it.”
After the situation is resolved, she asks a security guard who’s laughing, “Do we really need a sign that says, ‘no shoes, no shirt, no pants, no service?’”

So I’m wondering, why would you actually pick a fight and get security called on you over keeping your pants on? Is he that pressed for drama in his life? I’ll bet he’s telling his friends, “I’m badass thug yo. I got security called on my ass at the ER.”

Etomidate Reaction

Etomidate is a cool drug and equally as scary regarding how fast it works. One minute the person it talking, the next they’re snoring. We use it for “conscious” sedation. It lasts only minutes.
This guy had a hip dislocation. He’s had it before so he was use to the procedure. He was laughing and joking about things. We all filed in to get ready to put him out and reduce his hip.
“We got an entire crew, eh,” he joked. “You’re not expecting me to croak are ya?” He carefully regarded each one of us and pointed, “What’s your name and what do you do,” he asked of us in turn. He made sure we knew it was our job to make sure he didn’t remember anything. We all promised. He was impressed that two doctors would be there. One would reduce the hip, the other would monitor his airway.
“So I’ll get billed twice” the old man joked, “my pockets aren’t that deep.” We all went back and forth with him in a verbal joust. He’s one of those patients who is a joy to work with. He had to be in a lot of pain, but he wasn’t complaining. As we put a cannula on him for oxygen, he frowned but maneuvered his head to help us tuck the tubing behind his ears, “I can go to an oxygen bar and pay 20 bucks, but here it’s 2300 bucks. I hope it’s really clean. I’ve heard you people like to drug customers.” We promised him that it was the same grade and that we do drug customers. He laughed hard about that.
As the nurse pushed the etomidate, one of the doctors asked what his plans were that day before he had fallen.
“Hell,” the old man grumbled. You could see in his eyes he was lining up another joke, “I was going to go climb a…” He was out.
It happens that quickly. One doc climbed up on top of the bed and a tech kept a hand on his back to keep him from falling off the bed. He pulled, twisted, straightened his leg while the other doc pushed on the man’s pelvis with an occasional glance at the monitor. Even with the patient out, his heart rate elevated with the pain his conscious was unaware of. Everyone was confident his hip was back in place.
Suddenly, the oxygen sats fell and the monitor sounded. We glanced around and found his pulse ox was off of his finger. We like to check the obvious first.

The nurse attempted to put it on his finger but his hand fisted and confusion showed in her expression. “He’s rigid.”
Then he stopped breathing on his own.
“Bag ‘em,” one of the docs ordered. We applied a mask and began breathing for him. The nurse still fought with the pulse ox. His hands were too tightly closed. We needed to know if he was getting enough oxygen.
“How about his toe,” another nurse offered. The pulse ox was moved and he was at 100% saturation. Still, what was with his sudden sky high blood pressure which had jumped to 212 systolic? His jaw was locked closed. He wouldn’t lower his arms and he had urinated on himself.
“Is he seizing,” one doc asked the other.
“Possibly,” he said and moved his legs, “his legs are relaxed though.” The doctor folded his arms. “The etomidate should have worn off by now.” We were five minutes into things.
“Lets give him some ativan,” the other doc ordered.
“You want two,” the nurse asked upon returning.
“Just one. He’s already out.”
The other doc, still concerned about his pressure said, “I’ll bet the blood pressure is off because of his rigidity,” while demonstrating the straightened flexed arms of the patient. The other doc nodded with agreement.
The nurse pushed the ativan and the patient began relaxing. The etomidate was wearing off. “Mr. R__, are you with us?” He started breathing on his own so we pulled the bag off of him. The doc reached over and rubbed his sternum briskly.
Suddenly the patient’s head popped up off the pillow. He looked around the room.  He smiled and launched into his joking manner immediately. “It’s nice to see you all again,” he teased. “I guess I did alright?”
We all laughed at how quickly he came around. The doc then explained about his reaction to the drug. The patient didn’t remember a thing, which is what he asked of us. The way his head popped up so suddenly had me chuckling for nearly and hour afterwards.
It was a reaction neither of the docs had ever seen before in their combined 30 years of practicing. The medical books stated 50% have that type of reaction.

Just as long as the heart keeps going, everything is alright.

Wake Up

“CPR in progress, eleven year old male, ETA in two minutes. We’re putting him in two,” the charge nurse barked out.
“Alright,” the Dr. H said then looked at me.
I looked at him, “Did she say ‘eleven’?”
“Yep,” he responded and finished signing a prescription I had typed up for another patient. “They can go. I told her already not to drink while on this medication, but tell her once more then meet me in two.”

On the way to room two, I nearly collided the boy’s family in the hall. Father, mother, and daughter had just arrived and looked panicked. “My son was brought in an ambulance,” the father said with his daughter perched on his hip, who had a hand up to her knuckles in her mouth.
“Come with me,” I told him and led them to room two. “Wait right here, I’ll tell the doctor you’re here.” They nodded, and by now the daughter had her head on Dad’s shoulder, still with her hand in her mouth.

The boy had just arrived and hadn’t been moved to the bed yet. The paramedics had him on a machine which will do CPR. This keeps anyone from getting tired and delivers good compressions all the time at a desired rate. He was only eleven years old so we would pull out all the stops.

“Mom and Dad are just outside,” I announced in a pause during the medics report. The doctor nodded to me and continued listening as the team provided life support. They were perfectly in sync. It was rather quiet and sombre. Everyone had their game faces on. Everyone was at their best.

“He was at a friend’s house, playing video games. He stood up and collapsed, the friend said. No known medical history. He’s been asystole the entire time. He had CPR prior to our arrival on scene. We haven’t had a shockable rhythm at all,” the medic shook his head.
I peeked past the curtain and saw a social worker had already joined the parents and felt a little relief. The mother kissed her husband and took her daughter. She was going to the waiting room. She was unable to stand hearing the machines and the communication between the team. I’m sure hearing, “Still no pulse” was ripping her heart in half.
We worked him for quite some time before the doctor left the room to update the father, who was pale and looked about ready to faint. He remained standing though, next to a chair he was allowed to sit in if he chose. He looked like a man who would go through the fires of hell to be next to his dying son.

“We don’t have a pulse, he’s not breathing on his own. We’re doing everything we can,” the doctor told him. “Would you like to come in?” The father nodded. Some doctors won’t let friends or family in while we code a patient. This one does, especially for kids. He later explained that sometimes it helps the parents see that everything we can do is being done.

I grabbed the chair and carried it in for the father because he looked ready to collapse. But he went straight to the bedside, standing as close as he could without being in the way. Nothing would keep this man from his son, even the pain of watching him die before his very eyes. The disbelief which filled those eyes was heart wrenching. He took in his son, from head to toe, regarding every tube, every line and the machine which was compressing the son’s lifeless body, and took a deep breath after holding it for quite some time.

For over an hour, we coded this boy and never did get a rhythm. The doctor moved to the father’s side and put a hand on his shoulder. “We’ve done everything we can.” Knowing what he meant, the father glanced at the tile floor and nodded. The doctor pronounced the boy quietly.

We cleaned him up. The mother and daughter had been brought from the waiting room. They wanted to see him. Mom set the daughter down and slowly approached the bed. The daughter, maybe around three or four years old, regarded each one of us before seeing her brother on the bed. She jogged up to his side, hand still in her mouth, her blond pony tail swaying back and forth and grabbed his hand and tugged. “Wake up, Mike,” she asked of him. “Wake up. It’s time to wake up.” She waited quietly for a response. The mother moved to pick her up. More forcefully, the little girl said, “Wake up” and tugged her brother’s hand with all her might. She pressed her little round cheek to his hand and asked her mother, “Why won’t he wake up?”

Each of use filed out, wiping tears from our eyes. The social worker had to keep it together for quite some time after. She had to stay to make sure the family didn’t need anything before she could take a moment for herself.
Some days just…suck.
